Mail Order - You have several options for mail order.

• print our mail order form then mail or fax it to us.
• Utilize the electronic shopping cart then choose "mail order" for payment options during checkout. You will be emailed a copy of the order. Just mail in your payment and be sure to include the order number. These orders are held until payment is received.

If you choose to pay by check or money order, this must done via the mail and your check/money order must be included. Credit cards can be processed offline providing all the information on the order form is filled out. Fax and mailing addresses are located at the bottom of this page.

Checks MUST have a physical address either in the imprinted address section or written in at the top. Without a physical address, we can not process your check for payment. Also, your drivers license number must be written on the check. This is part of the TeleCheck process and not optional unfortunately.
